Lineage.  Organized as 1630 Support Squadron (Transport) on 1 Apr 1964.  Discontinued on 8 Jan 1966.  Consolidated (8 Jan 1966) with the 628 Military Airlift Support Squadron, which was constituted, and activated on 27 Dec 1965.  Organized on 8 Jan 1966.  Redesignated as: 628 Airlift Support Squadron on 1 Jun 1992; 628 Air Mobility Support Squadron on 1 July 1994; 728 Air Mobility Squadron on 15 March 2001.

Assignments.  322 Air Division, 1 Apr 1964-8 Jan 1966.  Military Airlift Command, 27 Dec 1965; 322 Air Division, 8 Jan 1966; 435 Military Airlift Support (later, 435 Tactical Airlift) Wing, 24 Dec 1968; 322 Airlift Division, 23 Jun 1978; 608 Military Airlift (later, 608 Airlift Support; 621 Air Mobility Support; 721 Air Mobility Operations) Group, 1 Apr 1992; 521 Air Mobility Operations Group, 4 Sep 2008-.
